The Patient Experience

The Reactivation Care Centres are a collaborative and innovative approach designed to help patients who no longer need acute care services, but often find themselves waiting for an alternate care facility, such as convalescent and long-term care.

A significant investment has been made at the Reactivation Care Centres to refresh and replace equipment so that patients and their families have access to the specialized care they need in a restorative setting.

We are excited to offer this opportunity to many of our patients and their families and believe the benefits will allow patients to resume more normal routines and restore their independence.

It’s all about delivering patient care in the most appropriate setting where patient privacy, dignity and safety are a top priority.

Benefits to Patients & Caregivers

  • A newly renovated facility that is a quiet, peaceful place for therapy, rehabilitation and activation supports relative to the pace and scale of an acute care hospital.
  • Specialized activation therapies in a setting designed to focus on the needs of the patient and support transition to home, or an alternate facility.
  • Patients will benefit from staff who are specifically equipped and trained to support patients with increased requirements for activation and support for daily living.
  • Patients enjoy single or 2-patient rooms in a renovated facility. Today, many patients transitioning out of hospital are in unconventional beds which, no matter the setting, are not optimal for the patient.
  • Patients will have access to specialized equipment, which reduces risks of hospital acquired pressure injuries and frequent falls.
  • Enhanced infection and prevention control measures provided by fixed ultra violet light technologies in patient washrooms.

Who is eligible for this specialized service?

Each hospital has developed criteria to assess which of its patients will be best suited to move to a Reactivation Care Centre. Those criteria will be clearly articulated at each hospital site and by the clinical teams responsible for their patients who are transitioning out of hospital.

Each participating hospital will occupy its own unit of the Reactivation Care Centre, treating its own patients, with clinical staff from that same hospital.
